All planets say "Explored" (not sure about final level - have no way of checking), got every chest, and every secret. 100% on your load screen would be chests, secrets and % of planets explored. God I hate 100% exploration, every save since my first has been 98/97% exploration, all chests and secrets. I'm not hunting down every nook and cranny

Basically it's a visual effect where the colors can't be focused into the same focal plane, so they appear as separate images oddly layered over top of each other. As seen in those screenshots, the effect can be rather harsh on the eyes. I too disable it in every game that has the setting. It's stupid.
If you open a chest or pick up an item, you will keep it even if you die. Dying in Fallen Order or any Souls game does NOT just revert to an old save, enemies just get their health back. This just isn’t as apparent in Fallen Order since there aren’t nearly as many items about, and not much reason to do suicide runs for items.
